ADMISSION PROCEDURE AND REQUIREMENTS
GRADE SCHOOL & HIGH SCHOOL DEPARTMENTS

1. Inquire at the Testing Center and fill-up the information sheet.

2. Pay the amount for application and testing fee at the Cashier's Office.

3. Secure an application form at the Testing Center.

4. Submit the following documents on or before the scheduled Entrance Examination.

a. Fully accomplished application form
b. Original and photocopy of Birth Certificate (NSO Copy)
c. Original and photocopy of Baptismal Certificate
d. Original and photocopy of Certificate of First Communion
e. Photocopy of available grades (Report Card).
f. Two (2) 1x1 recent identical pictures.
(Please write the name of pupil/student and school year at the back of each picture)
g. Two (2) Letters of Recommendation sealed and signed. (use Canossa Form),
one (1) from the Principal and one (1) from the Guidance Counselor or Classroom Adviser
h. Long folder

FOR HIGH SCHOOL SCHOLARSHIP APPLICANTS ONLY:
i. Photocopy of the latest Income Tax Return (ITR) or affidavit of non-filing of ITR
j. Scholarship Recommendation (use Canossa Form)

>> Only application with correct and complete information & requirements will be processed.
>> Original copy of documents will be returned. Submitted photocopied documents are for office file and they cannot be claimed.

5. Take the exam on the scheduled date and bring the following:

a. Examination Permit
b. Two (2) sharpened pencils with eraser
c. Blue or Black ballpen ( for Grade 3 - Year III)

6. Secure Notice of Acceptance/Reservation from the Testing Center. Pay the reservation fee to the Cashier's Office within the period stated in the Notice of Acceptance/Reservation. FIRST COME FIRST SERVED. This is non-refundable and non-transferable, but is deductible from the Tuition Fee for the coming school year.

Please bring the following on ENROLLMENT:

a. Original Report Card signed by the Principal
b. Official Receipt of Reservation fee

AGE REQUIREMENT:
Nursery  -  at least 3 years old by June
Kinder            -           4 years old by June
Prep               -           5 years old by June
Grade 1        -           6 years old by June
